Pepsinogen, a single-chain polypeptide of 375 amino acids and 42 kDa, is produced in gastric cells. Pepsinogen I and II, synthesized in different cell types, aid in diagnosing severe atrophic gastritis and stomach cancer. Measuring serum pepsinogens acts as a 'serological biopsy' for these conditions. In atrophic gastritis, low serum pepsinogen I levels (below 20 ng/ml) are highly specific, with sensitivity and specificity around 92% and 90% respectively. Low pepsinogen I levels also indicate gastric cancer risk, while a low pepsinogen I/pepsinogen II ratio suggests atrophic gastritis. Conversely, a low serum pepsinogen I level excludes duodenal ulcers.
